On Tuesday 9/19, the ICH convened for its third meeting of 2017.  At the meeting, the ICH reviewed and voted to adopt the 2017-2018 Winter Plan, which describes how the Continum of Care (member agencies and community providers) will coordinate to provide hypothermia shelter and services.  In addition to the Winter Plan, the meeting also highlighted the Coordinated Assessment and Housing Placement (CAHP) System for Singles.
